Sleep Issues and Low Energy: Medical Evaluation Beyond "Get More Rest"
Persistent sleep disruption and low energy are rarely the result of lifestyle alone. AvaOne’s structured six-month physician-led program evaluates the medical, metabolic, and lifestyle factors that determine whether real change is possible.
What "Sleep Issues" and "Low Energy" Actually Mean
Sleep issues and low energy are two of the most common complaints in primary care, and two of the most poorly served. Patients describe difficulty falling asleep, difficulty staying asleep, waking unrefreshed despite adequate time in bed, daytime fatigue that does not respond to more rest, or persistent low energy that limits the activities that matter to them. For many adults, these symptoms have been present for months or years, with conventional responses ranging from “get more sleep” to “try melatonin” to “talk to your doctor about a sleep aid.” None of these responses address the underlying drivers of the symptoms.
The clinical picture of sleep and energy issues is broader than the symptoms themselves. Underlying contributors include sleep apnea (frequently undiagnosed), thyroid dysfunction (which can produce both insomnia and fatigue), iron and other nutritional deficiencies, hormonal changes during perimenopause and menopause, chronic stress and cortisol dysregulation, blood sugar instability and insulin resistance, certain medications, and various medical conditions including cardiovascular and metabolic disease. The specific drivers vary significantly between patients, which means a generic approach to sleep and energy rarely produces sustainable improvement.
The clinical opportunity is identifying which underlying factors are at play for each individual patient. A patient whose primary issue is undiagnosed sleep apnea needs sleep study evaluation, not sleep hygiene advice. A patient whose primary issue is iron deficiency needs iron status testing and appropriate intervention, not melatonin. A patient navigating perimenopausal hormonal change needs different support than a patient whose energy issues are tied to chronic stress and cortisol elevation. The right evaluation determines the right intervention.
Why "Try Melatonin" and "Get More Rest" Are Not Working
The conventional medical response to sleep and energy concerns is typically inadequate for patients whose symptoms have persisted despite the standard advice. A patient mentions sleep issues during their annual physical. The physician suggests improving sleep hygiene, considers ordering basic bloodwork including a CBC and thyroid panel, and may discuss melatonin or other over-the-counter sleep aids. For many patients, this conventional response either does not identify the underlying issue or provides only temporary symptomatic relief.
The standard sleep aid medications, including prescription benzodiazepines and certain antidepressants used for sleep, can address symptoms in the short term but do not resolve underlying contributors and carry meaningful risks with long-term use. Melatonin can be useful for circadian rhythm disorders and jet lag but is not the right intervention for most chronic insomnia. Sleep hygiene advice (dark room, consistent bedtime, avoiding screens) is genuinely useful as a foundation but is rarely sufficient for patients whose sleep issues are driven by underlying medical or physiological factors.
The clinical evidence on sleep and energy is clear: when sleep issues persist despite reasonable lifestyle interventions, medical evaluation that identifies the underlying drivers is the most effective starting point. Sleep apnea is one of the most underdiagnosed conditions in adult medicine, affecting roughly one in four adults to varying degrees, with most cases undiagnosed. Thyroid dysfunction is similarly underdiagnosed. Iron deficiency in adult women is more common than typically recognized. The conventional system does not always have the time to investigate these underlying contributors. A structured evaluation that does have the time can change the trajectory.
How AvaOne Approaches Sleep Issues and Low Energy
At AvaOne, sleep issues and low energy are treated as symptoms with underlying medical drivers that warrant systematic evaluation. Every patient begins with a comprehensive Future Self Assessment that includes detailed sleep and energy history, screening for sleep apnea symptoms, complete medical history, current medications and supplements, lifestyle factors, and advanced lab work. The goal is to identify the specific underlying contributors for each individual patient.
The advanced lab work typically includes comprehensive thyroid evaluation (TSH, free T3, free T4, thyroid antibodies when clinically appropriate), iron status including ferritin (which captures iron stores more accurately than serum iron alone), vitamin D and B12 status, comprehensive metabolic panel including fasting insulin, inflammation markers, and when clinically appropriate, hormone panels. The clinical evaluation includes screening for sleep apnea using validated tools, with referral for sleep study when indicated. The result is a clinical picture that conventional primary care rarely produces.
The personalized plan addresses the specific drivers identified for each patient. For patients with positive sleep apnea screening, we coordinate referral for sleep study and ongoing care while addressing other lifestyle and metabolic contributors in parallel. For patients with thyroid dysfunction, iron deficiency, or other addressable medical contributors, we coordinate with primary care on appropriate treatment while addressing the broader lifestyle context. For patients whose primary issues are lifestyle and behavioral (sleep timing, stress, sleep environment, evening routines), we provide structured support for sustainable change.
The program is led by Jaimy Patel, MD, a board-certified internal medicine physician and certified health and wellness coach with training at a Harvard-affiliated teaching hospital, supported by Vanisha Patel, MD. The combination of internal medicine training and lifestyle medicine expertise brings the depth necessary to evaluate sleep and energy issues thoroughly.
The AvaOne approach addresses sleep and energy concerns through medical evaluation and structured lifestyle intervention. We do not provide sleep medicine specialty care including sleep studies, CPAP titration, or treatment of severe sleep disorders. For patients whose evaluation indicates sleep apnea or other conditions requiring sleep medicine specialty care, we coordinate referral to appropriate specialists while addressing lifestyle factors in parallel.
The Right Program for Patients With High Blood Pressure
For most adults with persistent sleep issues or low energy that have not responded to basic sleep hygiene and conservative measures, the AvaOne Lifestyle and Wellness Coaching Program is the right starting point. This is a structured six-month physician-led program that includes the comprehensive medical evaluation, advanced lab work, personalized plan, bi-weekly coaching sessions, mid-program reassessment with repeat labs, and direct messaging access described above. The program is $999 onboarding plus $499 per month for five months.
For patients whose sleep issues are accompanied by significant cardiovascular risk, family history of heart disease, or elevated cardiovascular markers, the Vitality Program may be the more appropriate starting point or a complementary addition. Sleep apnea in particular is closely associated with cardiovascular risk, and patients with both concerns may benefit from cardiology-led care. Your physician will recommend the best fit during your Future Self Assessment.

I came to AvaOne because I had a heart condition and I was tired of being told to 'just watch my diet.' Dr. Chirag actually ran advanced testing I'd never had before, identified my real risks, and built a plan around my life. Six months into the Lifestyle Coaching Program, my cholesterol is down, I'm sleeping better, and I finally feel like I'm in control of my health.

I struggled with my weight for years and nothing worked. The team at Ava One didn't just hand me a prescription — they looked at my hormones, my blood sugar patterns with a CGM, and my stress levels. The whole approach was different from anything I'd tried before. I've lost 38 pounds and I actually understand my body now.
— S.K., Woodbridge, NJ
After years of living with chronic knee pain and being told my only options were injections or surgery, I found Ava One's STEMWAVE therapy. It was completely non-invasive and I started noticing improvement after just a few sessions. I'm back to walking and playing with my grandkids pain-free.
— D.P., Metuchen, NJ
I was dependent on Ambien for almost five years and no doctor ever tried to help me get off it. Dr. Jaimy took the time to understand why I wasn't sleeping — the stress, the habits, everything. Through breathwork, lifestyle changes, and real support, I'm now sleeping naturally for the first time in years. This practice is truly different.

Frequently Asked Questions About Sleep Issues and Low Energy
Persistent low energy despite adequate sleep duration is one of the most common patient concerns we hear. The underlying drivers vary significantly between patients but commonly include undiagnosed sleep apnea (which fragments sleep without the patient being aware), thyroid dysfunction, iron deficiency (particularly in adult women), vitamin B12 deficiency, blood sugar instability, chronic stress and cortisol dysregulation, certain medications, and various medical conditions. The conventional response of “try to get more sleep” often misses these underlying contributors entirely. A comprehensive medical evaluation that includes appropriate lab work and sleep apnea screening is the most useful starting point because it identifies what is specifically driving your fatigue, which then determines what intervention is likely to help.
Persistent low energy can be a symptom of various medical conditions but is rarely a primary diagnosis on its own. Chronic Fatigue Syndrome (also called Myalgic Encephalomyelitis or ME/CFS) is a specific clinical diagnosis that requires evaluation by a specialist with expertise in that condition. Persistent fatigue can also be a symptom of thyroid dysfunction, iron deficiency, sleep apnea, depression, cardiovascular disease, autoimmune conditions, and various other medical conditions that require appropriate evaluation. The AvaOne Future Self Assessment is designed to identify the contributors to your fatigue through clinical evaluation and lab work, with referral to appropriate specialists when conditions requiring specialty care are identified.
AvaOne does not provide sleep medicine specialty care including sleep studies, CPAP titration, or treatment of diagnosed sleep apnea. We do screen for sleep apnea symptoms during our evaluation and coordinate referral to sleep medicine specialists when sleep apnea is suspected or confirmed. For patients with diagnosed sleep apnea who are using CPAP or other treatment, AvaOne’s lifestyle medicine program can address the broader lifestyle and metabolic factors that contribute to overall health, complementing the sleep medicine specialty care. Sleep apnea is closely associated with cardiovascular risk, weight loss resistance, and metabolic dysfunction, all of which are within the scope of AvaOne’s program.
