AvaOne Hybrid Care is the delivery model that runs across all three AvaOne programs. Patients begin care in person at our Edison, NJ clinic with a Future Self Assessment, then continue much of their ongoing care through secure telehealth across New Jersey. Lab reviews, coaching check-ins, progress conversations, and follow-up visits can happen virtually. In-person visits are reserved for evaluations, biometric updates, and treatments that genuinely require them.
The model was built around a simple problem. Most premium preventive care practices require patients to drive to a clinic for every interaction, which limits how many adults can realistically engage with that depth of care. At the other extreme, fully virtual telehealth wellness coaching can lack the depth and trust of an in-person physician relationship. AvaOne Hybrid Care sits in between, with in-person care anchoring the relationship and virtual visits supporting the ongoing work.
Patients enrolled in the Vitality Program and the Lifestyle and Wellness Coaching Program both use the Hybrid Care model. StemWave Therapy is in-person only, since the acoustic wave treatment must be physically applied at the clinic.

Most ongoing AvaOne care can be delivered through secure telehealth. This includes:

Following your assessment, your physician builds a personalized care plan and recommends which AvaOne program fits your situation. The plan includes a schedule of in-person and virtual visits over the coming twelve months.
Continue Care Through Telehealth Between In-Person Visits
Routine follow-up visits, lab reviews, coaching sessions, and progress check-ins are delivered through secure telehealth across New Jersey. Virtual health coaching in New Jersey is scheduled at your convenience and conducted via HIPAA-compliant secure video.
Return In Person for Periodic Assessments
Quarterly in-person visits for Vitality Program members, two scheduled in-person check-ins during the six-month Lifestyle and Wellness Coaching Program, and any clinically necessary physical evaluations or procedures happen at our Edison clinic. The relationship stays grounded.

New Jersey Coverage and Telehealth Eligibility
Telehealth visits are available to New Jersey residents only. This is a state licensing requirement, not an AvaOne policy. Our physicians are licensed to practice medicine in New Jersey, and that license covers telehealth visits with patients physically located in the state at the time of the visit.
Patients who live outside New Jersey are welcome to travel for in-person assessments and care at our Edison clinic. Ongoing telehealth follow-up cannot be provided across state lines.
